Why is the knee replacement painful or failing?

What Is Revision Knee Replacement?

Revision knee replacement is surgery performed to treat a significant problem involving a previous knee replacement.

It may involve:

  • Replacing one component
  • Replacing several components
  • Removing the entire previous prosthesis
  • Managing bone loss
  • Using stems, augments or more constrained implants
  • Treating infection
  • Reconstructing soft-tissue or ligament problems

The procedure must be tailored to the cause of failure.

Why Can a Knee Replacement Become Painful?

Potential causes include:

  • Infection
  • Implant loosening
  • Instability
  • Malposition
  • Malalignment
  • Stiffness
  • Polyethylene wear
  • Fracture around the implant
  • Patellar problems
  • Extensor mechanism problems
  • Referred pain from the hip or spine

A painful knee replacement should therefore be investigated systematically rather than assumed to be “worn out.”

How Is a Painful Knee Replacement Evaluated?

Assessment begins with history, examination and appropriate weight-bearing radiographs.

Additional investigations are selected according to the suspected cause and may include comparison with previous imaging, inflammatory markers, joint aspiration, CT or other specialized tests.

The goal is to establish the most likely mechanism of failure before revision surgery is planned.

How Is Periprosthetic Joint Infection Evaluated?

Periprosthetic joint infection is one of the most important diagnoses to consider in a painful or failing knee replacement.

Evaluation may include:

  • Clinical examination
  • ESR and CRP or other inflammatory markers when appropriate
  • Joint aspiration
  • Synovial-fluid cell count and differential
  • Microbiological cultures
  • Additional synovial or diagnostic testing in selected cases

No single blood test, aspiration result or culture should necessarily be interpreted in isolation.

Normal inflammatory markers reduce suspicion in some settings but do not absolutely exclude all prosthetic joint infections.

What Is Aseptic Loosening?

Aseptic loosening occurs when an implant loses stable fixation to bone without infection being the primary cause.

Patients may develop:

  • Pain
  • Reduced function
  • Progressive radiographic changes
  • Bone loss

Before labeling loosening as aseptic, infection should remain part of the differential diagnosis when clinically appropriate.

Revision usually requires removal of the loose components and reconstruction of stable fixation.

What Is Instability After Knee Replacement?

A replaced knee still depends on appropriate alignment, implant position and soft-tissue balance.

Instability can occur if the knee is excessively loose during:

  • Extension
  • Flexion
  • Mid-flexion
  • Varus or valgus loading

Patients May Experience:

  • Giving way
  • Recurrent swelling
  • Difficulty on stairs
  • Lack of confidence in the knee

Treatment depends on the mechanism of instability.

What Is Knee Stiffness After Replacement?

Before revision for stiffness, the cause should be carefully assessed.

What Is Bone Loss in Revision Knee Surgery?

Removal of previous implants and the underlying disease process can leave areas of deficient bone.

Bone-loss reconstruction aims to obtain durable fixation in the remaining bone while restoring joint position and stability.

Reconstruction may require:

  • Metal augments
  • Stems
  • Metaphyseal cones
  • Sleeves
  • Bone graft
  • More constrained components

The amount and location of bone loss influence implant selection. No single reconstruction system is universally superior, and not every listed option is needed together.

What Does “Constraint” Mean?

In primary knee replacement, the patient's ligaments provide much of the stability.

In revision surgery, ligaments may be deficient or damaged.

More constrained implants can provide additional mechanical stability when the native soft tissues cannot.

The level of constraint should match the actual stability requirements of the knee.

Greater constraint can provide mechanical stability when ligament function is inadequate, but it does not replace the need for secure implant fixation and appropriate reconstruction.

What Is a Hinged Knee Replacement?

A rotating-hinge or hinged reconstruction may be considered in selected very complex revision cases.

Potential indications include:

  • Severe ligament deficiency
  • Major bone loss
  • Certain tumor reconstructions
  • Other situations where standard implants cannot provide adequate stability

A hinge should not be considered automatically necessary for every revision.

What Happens During Revision Knee Replacement?

The procedure can include:

  • Removal of previous components
  • Assessment of fixation
  • Management of scar tissue
  • Assessment of bone defects
  • Restoration of alignment
  • Ligament and stability assessment
  • Reconstruction using revision components

The operation is often more extensive than primary knee replacement.

How Is an Infected Knee Replacement Treated?

Treatment strategy depends on whether the infection is acute or chronic, symptom duration, implant fixation, soft-tissue condition, organism when known, bone loss, previous procedures, patient health and the ability to perform adequate debridement.

Options may include:

  • DAIR in selected cases
  • One-stage exchange in selected cases
  • Two-stage exchange
  • Other individualized strategies

Two-stage revision is an important strategy for selected chronic, complex or difficult-to-treat infections, but it is not a universal gold standard. Infection treatment must be coordinated with appropriate microbiological and medical management.

What Is Two-Stage Revision?

In a two-stage strategy, the infected prosthesis is removed during the first operation, with thorough debridement, appropriate cultures, an antibiotic-loaded spacer when appropriate and an individualized antimicrobial strategy.

After treatment of infection and appropriate reassessment, definitive revision knee replacement may be performed later when the overall clinical situation supports reconstruction.

The exact sequence varies according to the individual case.

How Long Does Revision Knee Recovery Take?

Recovery varies according to:

  • Cause of revision
  • Bone loss
  • Soft-tissue condition
  • Infection
  • Number of previous operations
  • General health
  • Complexity of reconstruction

Revision recovery may be slower and less predictable than recovery after primary knee replacement.
