Acute complex injuries
Some injuries require urgent stabilization, appropriate imaging, soft-tissue assessment and coordination with other specialties before definitive orthopedic reconstruction is planned.
Fracture nonunion
Delayed or failed healing requires consideration of fracture stability, alignment, biology, patient factors and previous treatment. The assessment seeks the reasons healing has not progressed as expected.
Fracture-related infection
Infection after fracture treatment is a distinct problem, although nonunion and fracture-related infection may coexist. Assessment may include clinical findings, imaging, laboratory tests and microbiology as appropriate.
Implant retention or removal
Implants are not automatically removed in every fracture-related infection or nonunion. The decision depends on fracture stability and healing, the infection strategy, implant function and the wider reconstruction plan.
Pelvis and acetabulum injuries

Staged reconstruction
Some complex cases require staged treatment or more than one operation. Timing depends on the injury, soft tissues, infection status, stability, associated injuries and the person’s overall condition.
When urgent trauma care is needed
Seek urgent trauma care for an open fracture, major deformity, a cold or pale limb, new major numbness or weakness, severe uncontrolled pain, a tense rapidly worsening limb that raises concern for compartment syndrome, or major trauma with inability to bear weight.
Does every complex fracture need surgery?
No. Treatment depends on the fracture pattern, stability, displacement, soft tissues, overall health and the individual circumstances.
Are implants always removed when infection is suspected?
No. Retention or removal is decided as part of an individualized infection, stability and reconstruction strategy.
