What Is Revision ACL Reconstruction?
Revision ACL reconstruction is surgery performed after a previous ACL reconstruction has failed to provide adequate stability or has developed another significant problem.
Revision surgery is different from primary ACL reconstruction because the knee has already undergone:
- Bone tunnel creation
- Graft placement
- Fixation
- Surgical alteration of normal anatomy
Associated meniscus or cartilage damage may also have progressed.
Planning therefore requires careful analysis of the previous operation.
What Does a Failed ACL Reconstruction Feel Like?
Patients may experience:
- Recurrent giving way
- Instability during sport
- Reduced confidence in the knee
- Recurrent swelling
- Pain
- Difficulty returning to activity
- New injury after reconstruction
However, not every painful knee after ACL surgery represents graft failure.
Pain, stiffness, meniscus injury, cartilage damage or other conditions may produce symptoms even when the graft remains structurally intact.
Why Does an ACL Reconstruction Fail?
Failure can occur for several reasons.
Potential contributors include:
- New traumatic injury
- Graft rupture
- Technical factors
- Inappropriate tunnel position
- Tunnel widening
- Fixation problems
- Biological failure
- Unrecognized additional ligament instability
- Meniscus deficiency
- Limb alignment problems in selected patients
- Excessive posterior tibial slope in selected complex cases
- Infection
More than one factor may be present.
Revision planning should therefore identify the failure mechanism rather than focus only on replacing the graft.
How Is a Failed ACL Reconstruction Evaluated?
Evaluation begins with a detailed history.
Important information includes:
- Original injury
- Date and type of reconstruction
- Graft used
- Previous rehabilitation
- Whether stability was ever restored
- New injuries
- Current instability
- Pain and swelling
- Previous infection or wound problems
Clinical Examination May Assess:
- Lachman test
- Pivot shift
- Collateral ligaments
- Posterolateral stability
- Range of motion
- Menisci
- Alignment
- Strength
Imaging is then selected according to the suspected problem.
What Imaging May Be Required?
Evaluation can include:
X-rays
X-rays help assess:
- Previous tunnels
- Fixation devices
- Alignment
- Arthritis
- Other bony abnormalities
MRI
MRI can provide information regarding:
- ACL graft
- Menisci
- Cartilage
- Other ligaments
- Joint inflammation
CT
CT can be particularly useful for detailed assessment of previous bone tunnels.
It may help determine:
- Tunnel position
- Tunnel diameter
- Bone defects
- Relationship between previous and proposed tunnels
The required imaging depends on the individual revision problem.
Why Is Tunnel Position Important?
An ACL graft must be positioned appropriately to reproduce ligament function.
If previous tunnels are substantially malpositioned, the graft may not control knee stability effectively.
Poorly positioned tunnels can also interfere with placement of new anatomical tunnels during revision surgery.
Revision planning must therefore determine whether previous tunnels can be reused, avoided, or need staged grafting to achieve correct placement and secure fixation; previous tunnels do not automatically require bone grafting.
What Is Tunnel Widening?
Bone tunnels can enlarge after ACL reconstruction.
The degree of widening varies.
Significant tunnel enlargement can make secure placement of a revision graft more difficult.
When bone stock is inadequate for reliable fixation or correct tunnel placement, staged bone grafting may be required before definitive revision reconstruction. There is no universal tunnel-diameter cutoff: the one-stage or two-stage decision is individualized according to tunnel size, position and overlap, available bone stock, planned fixation and infection status.
Can Revision ACL Be Done in One Operation?
Yes, in selected cases.
A one-stage revision may be possible when:
- Previous tunnels do not interfere significantly with the planned reconstruction
- Bone stock is adequate
- Secure graft fixation can be achieved
- Infection is not present
- Associated problems can be addressed appropriately
The decision should be made after appropriate imaging and surgical planning.
When Is Two-Stage Revision ACL Surgery Needed?
A staged approach may be appropriate when:
- Previous tunnels are substantially enlarged
- Tunnel position prevents appropriate revision tunnel placement
- Bone defects require reconstruction
- Infection is suspected or confirmed
- Other factors make immediate definitive reconstruction inappropriate
In a staged strategy, the first operation may address previous graft material, fixation, tunnels, infection or bone defects.
Definitive ACL reconstruction is performed later after the knee is ready for the next stage.
What Happens When Infection Is Suspected?
Infection after ACL reconstruction is uncommon but important.
When infection is suspected, evaluation may include:
- Clinical assessment
- Blood investigations
- Joint aspiration when appropriate
- Microbiological cultures
- Imaging
Treatment priorities change when infection is present.
Eradication of infection generally takes priority over immediate ligament reconstruction.
In complex infected cases, staged treatment may be required.
What About Previous ACL Fixation Devices?
Previous screws, buttons, anchors or other fixation devices may remain in the femur or tibia.
Not every implant must automatically be removed.
Removal may be required when the device:
- Interferes with new tunnel placement
- Is loose
- Is associated with infection
- Prevents appropriate reconstruction
Otherwise, selected existing fixation may sometimes remain in place.
Why Are the Menisci Important in Revision ACL?
The menisci contribute to knee stability as well as load distribution.
Meniscal deficiency can increase mechanical demands on an ACL graft.
Revision assessment should therefore determine:
- Whether the medial meniscus is intact
- Whether the lateral meniscus is intact
- Whether a previous repair has healed
- Whether a new tear is present
- Whether meniscal tissue can be preserved or repaired
Revision ACL surgery should not focus on the ACL in isolation.
What About Other Ligaments?
Persistent instability can occur when injuries to other stabilizing structures have been overlooked or have become chronic.
Assessment may therefore include:
- Medial collateral structures
- Lateral collateral ligament
- Posterolateral corner
- Posterior cruciate ligament
- Rotational stability
In selected patients, addressing associated instability may be important to protect the revision ACL graft.
Do Alignment and Tibial Slope Matter?
In selected complex or multiply failed ACL reconstructions, bony anatomy can contribute to abnormal graft loading.
Assessment may include:
- Coronal limb alignment
- Posterior tibial slope
- Previous bony procedures
Corrective osteotomy is not routinely required for every revision ACL.
It may, however, become relevant in carefully selected complex cases.
What Graft Is Used for Revision ACL?
Graft choice depends on:
- Previous graft
- Available autograft tissue
- Patient age
- Activity
- Tunnel strategy
- Associated procedures
- Surgeon assessment
Options May Include:
- Quadriceps tendon autograft
- Patellar tendon autograft
- Hamstring tendon autograft when appropriate
- Selected allograft tissue
There is no single graft choice appropriate for every revision.
What Happens During Revision ACL Surgery?
The procedure depends on the reason for failure.
Possible components include:
- Arthroscopic assessment
- Removal of failed graft tissue
- Treatment of meniscus injuries
- Cartilage assessment
- Removal of interfering fixation
- Creation of appropriate revision tunnels
- Graft preparation and fixation
- Treatment of associated ligament instability
In staged cases, definitive ACL reconstruction may not occur during the first operation.
Is Recovery Different from Primary ACL Reconstruction?
Revision rehabilitation follows many of the same principles as primary ACL rehabilitation but may need to be more individualized.
Recovery can be affected by:
- Meniscus repair
- Cartilage procedures
- Additional ligament reconstruction
- Bone grafting
- Previous stiffness
- Multiple previous surgeries
The objective should be safe progression rather than simply reproducing a standard timeline.
When Can I Return to Sport After Revision ACL?
Return to sport after revision reconstruction should be approached cautiously.
Assessment may include:
- Knee stability
- Strength
- Limb symmetry
- Functional testing
- Movement quality
- Sport-specific ability
- Psychological readiness
Previous reconstruction failure should be considered when evaluating future risk.
Return should not be determined by time alone.
Does a Torn ACL Graft Always Need Revision Surgery?
No. Treatment depends on instability, activity requirements, associated injuries and individual circumstances.
Can My Old ACL Tunnels Be Used Again?
Sometimes. Their position, size and relationship to the planned reconstruction must be evaluated.
Why Would I Need Bone Grafting Before Revision ACL?
Bone grafting may be necessary when previous tunnels are too large or poorly positioned to allow secure anatomical revision reconstruction.
How Long Between Stages of Revision ACL?
Timing depends on bone healing, infection status when relevant, knee condition and the reconstruction plan. It should be individualized.
Can Meniscus Repair Be Done During Revision ACL?
Yes, when a repairable meniscus tear is present and treatment is appropriate.
Is Revision ACL More Difficult Than the First Operation?
It can be. Previous tunnels, fixation, bone loss and associated injuries make planning more complex.
Can I Return to Sport After Revision ACL?
Return may be possible after appropriate reconstruction and rehabilitation, but individual prognosis depends on the knee, associated damage and functional recovery.
